BONE TUMORS

Abstract
Primary spinal tumors are relatively rare nosologies in the structure of oncopathology which constitute about 0,4% of all malignant neoplasms. Despite the obvious successes of recent decades in the treatment of bone sarcoma, the results of therapy for primary spinal tumors due to anatomical localization, difficulties in diagnosing and the complexity of the surgical intervention remain unsatisfactory. Our article contains a review of the literature devoted to the main primary malignant tumors of the spinal column.

Abstract
Total joint replacement has long been the orthopaedic gold standard of treatment of tumor lesions of the bone. However, in patients with locally advanced tumors, there is an increased risk of local recurrences due to dissemination and involvement of surrounding anatomical structures. Soft tissue deficiency can cause delayed wound healing. To prevent these complications, extraarticular resection (EAR) techniques are used, followed by reconstruction with endoprostheses and soft tissue plastic. Analysis of the results shows a reduction in the level of the described complications, as well as satisfactory functional results on the MSTS scale.

Abstract
The article describes the main aspects of the application of computer technologies and 3D-printing in the treatment of patients with tumors of pelvic bones. The presented review of the literature, the types of prototyping of surgical procedure and stages of the introduction of technologies are considered.

Abstract
Breast sarcomas are spectrum of heterogeneous sarcomas in breast and the optimal treatment is still under discussion. We retrospectively reviewed 51 patient with BS between 1999 and 2014 in N.N. Blokhin Russian Cancer Research Center Ministry of Health of the Russian Federation. With median follow up of 86,4 months, 43,14% of patients (22/51) developed local recurrence and metastasis. Five year recurrence free survival and OS rate were 37,5 and 56,2%, respectively. Disease stage and local recurrence were indicated as unfavorable prognostic factors. Surgery remains a crucial treatment for breast sarcomas. Mastectomy is not routinely necessary if clear margin could be achieved by local excision.

Abstract
Background. This study was conducted to avaluate various risk factors that may influence on soft tissue sarcoma recurrences on the basis of clinical and morphological representations and quantitative statistical analysis. Materials and methods. Treatment results of 168 patients with primary soft tissue sarcomas, who received multicomponent induction and surgical treatment were analyzed. Based on the obtained data, a table of clinical and morphological factors for the prediction of sarcoma recurrence was compiled, the factors were included into groups that exert an influence on the occurrence of relapse - strong, medium and weak. 19 independent variables were selected and on their basis the possibility of relapse was analyzed, their significance as risk factors for the recurrence of soft tissue sarcoma was assessed. Results. It has been established that it is necessary to take into account the fact that surgical operation in case of sarcoma should be planned, while treatment should be carried out in specialized centers. The obtained results allowed to determine the criteria determining the quality of surgical intervention. The choice of the boundaries of resection should be based on the notions of tumor growth and the degree of malignancy, the margin of resection should be characterized collectively by surgeons and pathomorphologists. It has been established that the margin of resection (negative or positive, R0 or R1) is a more accurate prognostic factor than its characteristic in mm. Conclusion. Most accurate factors that influenced the occurrence of relapse in patients with soft tissue sarcomas included: tumor malignancy degree; margin of resection; tumor pathomorphosis; relationship of the tumor with the bone. The results of statistical calculations confirmed the data of clinical and morphological analysis.

Abstract
The modern achievements of oncology, basing on the new therapeutic technologies of combined treatment of the tumors, are widely connected with perfect morphological verification of the tumoral lesion. Percutaneous biopsies of the spine is modern mini invasive way of getting a morphological verification of the lesion, affording to get diagnosis in the short time. The article presents 15years experience of percutaneous biopsies of the spine, performed in one institution from 2001 to 2016.

Abstract
Objective. To develop a concept that allows complete rehabilitation of patients with postmastectomy lymphostasis. Often in patients with secondary lymphatic edema of the hand after complex treatment of breast cancer breast is completely resected. The optimal approach in this case is the maximum restoration of the former anatomy for a minimum number of surgical interventions. Materials and methods. 25 patients with postmastectomy lymphostasis are included in the study. Inclusion criteria: absence of breast and presence of lymphostasis of the hand. All patients underwent BRANT operation (Breast Reconstruction And Node Transplantation), which implies microsurgical auto transplantation of inguinal lymph flap in the axillary region and use any of the available autologous grafts or combination of materials and silicone implants for breast reconstruction volume. Results. The average regression of the edema after 1 year after surgery was 70%. Quality of life and social activity of patients increased significantly. All patients of working age were able to return to work after 1-2,5 months after the operation. Conclusions. BRANT allows for a minimum number of surgical interventions to restore the attractive look of breast and to achieve good results in the treatment of hands lymphoedema. An important aspect of this approach is full fledged social rehabilitation of patients.

Abstract
Meningeal hemangiopericytomas, or solitary fibrous tumors, are rare tumors whose treatment tactics remain controversial, but the accumulated clinical data indicates the need for a radical removal of the primary tumor followed by a course of external-beam radiation therapy on the surgical bed in adequate total focal doses to achieve the best disease control.
