EDITORIAL
Spinal cord compression syndrome is a threatening complication of metastatic lesions of the spine in patients with cancer. No specific treatment or ineffectiveness of such treatment lead to the development of sensory disturbances and motor functions of limbs, pelvic organ dysfunction, severe pain syndrome. Timely conducted course of external beam radiotherapy in separate mode or complex therapy prevents the patient’s disability, and significantly improve the quality of life. This article provides two examples of the successful treatment of patients with spinal cord compression syndrome with the use of external beam radiation therapy as part of comprehensive treatment.
SOFT TISSUE SARCOMAS
117 patients with locally advanced STS treated in MRRC were analyzed. In group 1 were included 72 patients who received preoperative thermo-chemo-radiotherapy (ТCRТ) and surgery (S). The group 2 consisted of 45 patients treated with preoperative chemo-radiotherapy (CRТ) followed by S too. The locoregional-recurrence-free-survival (LRFS), metastasis-free survival and overall survival rates (93, 74, 88,5%) at 5 years were higher in groups with ТCRТ compared to the CRT (72,5, 59, 63%). Local hyperthermia didn’t increase an incidence of postoperative complications.
TUMORS OF THE SKIN
It was performed an analysis of published data on possible methods of isolated regional perfusion (IRP) in the treatment of locally advanced extremity melanoma. It is shown that the method is becoming more common as superior to other methods in the treatment of tumors of this embodiment. It was found that when using IRP average five-year survival without signs of the disease is 39%, while in patients underwent IRP was unresectable tumor with a poor prognosis. In regard to safety performance it demonstrated that IRP is rarely accompanied by severe manifestations of local or systemic toxicity. These data confirm the safety and the possibility of using IRP method in a clinical setting. The study of patients after IRP quality of life showed an increase in its level after treatment by this method. There are reports on the efficacy of the method in combination with surgical treatment of the tumor. However, to date there is no evidence that adjuvant IRP extends the life of patients with melanoma, indicating a need for further research possibilities of this method.
Pediatric Oncology
Naevus flammeus or Port wine stain (Pws) – is an inherent capillary malformation, occurs every 3–4 children per 1,000 live births. The gold standard of PWS treatment is a pulsed dye laser (Pdl) with a wavelength of 585–595 nm. according to modern international standards laser treatment should begin in infancy and due to the painful procedure general anesthesia is recommended. A significant part of the parents applies to general anesthesia with prejudice, exposing the baby pain impact shocks (surgical stress). Multiple modern general anesthesia by sevoflurane in outpatient PWS laser treatment involves minimal and short-term cognitive impairment. This data describes the drug and psychotherapeutic correction of cognitive impairment.
EXPERIMENTAL ONCOLOGY
The results of studies of interleukin 16 (IL-16) and vascular endothelial growth factor (VEGF) by ELIsa in the serum of patients with primary malignant tumors of bone and healthy age-matched to identify the possible correlation with the histological structure of the tumor and prognosis of the disease were presents. It is shown that the detection rate of IL-16 in serum in bone tumors was 93%, no significant differences in levels of IL-16 in considering the histological structure of tumor is not revealed. relationship between the size of the primary tumor and the content of IL-16 in serum were not found. overall 3- and 5-year survival of patients with malignant bone tumors content IL-16 in serum >33.0 pg/ml was significantly lower than patients with levels of IL-16 <33.0 pg/ml. osteosarcoma overall 5-year survival in patients with high IL-16 in serum was 1.6 times, with Ewing’s sarcoma in 1.7 times, chondrosarcoma – 1.8 times lower than in patients with the content IL-16 in serum <33.0 pg/ml. malignant bone tumors, VEGF levels were significantly higher than in borderline and benign tumors, statistical analysis of significant differences in the levels of VEGF in view of the morphological structure of the primary tumor has not revealed. maximum values VEGF marked with periosteal osteosarcoma, minimum – at paraosteal osteosarcoma – osteosarcoma with a favorable prognosis. when the content of IL-16 >33.0 pg/ml, which to our knowledge has been less favorable prognosis than patients with lower levels of IL-16, VEGF levels in the serum in all morphological variants of the structure of the primary tumor were higher than with IL-16 levels <33.0 pg/ml, but the differences were not statistically significant. In healthy people marked inverse relationship between the levels of VEGF and IL-16 in serum (r=–0.34), in benign tumors – a direct correlation (r=0.46), which is possible due to the peculiarities of the growth of the tumors. Initially high rates of IL-16 to meet the high values of VEGF in serum (p<0.05). Indicators of overall 3- and 5-year survival in patients with bone sarcomas levels of VEGF >493 pg/ml in serum were significantly lower than among those with low values of VEGF (<493 pg/ml; p<0.05). osteosarcoma no significant differences in the 3- and 5-year overall survival for baseline values of VEGF in blood serum. when Ewing sarcoma and chondrosarcoma of bone were significant differences in the 3- and 5-year overall survival, which depended on the baseline VEGF. Greatest figures 3 and 5-year overall survival observed in patients with bone sarcomas containing IL-16 <33 pg/ml VEGF <493 pg/ml and at least baseline IL-16 <33 pg/ml VEGF >493 pg/ml (p<0,05).
ORTHOPAEDIC PATHOMORPHOLOGY
Chordomas, low-grade chondrosarcomas, metastatic carcinomas share many histological features, generating at times, considerable diagnostic difficulty and, not infrequently, requiring immunohistochemical analysis for appropriate classification. In an effort to clarify the diagnosis and differentiation with other cartilaginous tumors, samples from 58 patients with sacrum chordoma were examined by immunohistochemistry with a panel of antibodies. The panel included antibodies to cytokeratin (CK), epithelial membrane antigen (EMA), D2-40, GFAP (glial fibrillary acidic protein), NSE (neuron-specific enolase), chromogranin, TTF-1 (thyroid transcription factor), PSA, CDX-2, mammaglobin. The conventional chordoma stained for CK (16/16) EMA (13/16), NSE (12/16) and was negative for chromogranin, GFAP. Specific markers were helpful in differential with metastatic carcinomas. Chordomas are difficult to excise because of their location and intimate relation to critical structures. Two cases of dedifferentiated chordoma were described.
REAR CLINICAL CASES
Melanoma metastases in the brain are frequent development of cancer process. Cerebral metastases of melanoma are with hemorrhage in half of all cases. Bleeding in the brain ventricles of parenchymal metastases are rarely observed. the description of the clinical observation of the patient with satisfactory results of surgical treatment of cerebral metastasis of melanoma in the right parietal lobe, complicated by bleeding in the development of the right lateral ventricle.
In these article we presented a clinical case of Stewart-treves syndrome treatment. Base of this syndrome is lymfangiosarcoma presented in chronicle lymphedema region, most common after radical mastectomy. aggressive disease with fast distance metastases. Surgery is the method of choice for treatment. Prognosis is bad.
